# Website-refresh intake form Version 1, 29 October 2026. Copyable questions with a fictional completed example. Use for a service engagement, not medical, financial or legal intake. Request only information needed to assess and start the work. | Question to copy | Fictional answer | What the delivery owner does with it | | --- | --- | --- | | What should change, and why now? | Refresh five service pages before December's campaign | Put the business objective in the brief; do not promise campaign results | | What is included and excluded? | Five existing pages, two review rounds; no ecommerce or copywriting | Confirm scope against the signed agreement | | Who approves scope and deliverables? | Rowan Lee | Verify authority and name the approval task owner | | Who supplies content and access? | Casey Morgan | Give each required input an owner and target date | | What date matters and what depends on it? | Reviewed build target 20 Nov; content and CMS access by 5 Nov | Separate the desired date from the accepted delivery plan | | What is the agreed budget or fee status? | USD 6,000 agreed; deposit status unconfirmed | Keep agreement and payment evidence distinct | | Which systems and materials are involved? | Existing CMS, brand folder, five copy documents | Request named invitations; check access rather than collecting passwords | | How should reviews be handled? | Rowan sends one consolidated response per round | Record the draft version and actual response | | What would prevent kickoff? | Missing content owner or unsigned scope | Leave the onboarding gate open and name the next action | | What happens after delivery? | Casey maintains content; support scope to be confirmed | Resolve before final handover | ## From answers to onboarding Create one account and the two people, then the delivery project. Put confirmed scope in the brief and keep uncertain answers under Open questions. Make the access request a task with a due date and a named owner. The intake response is an input to review; it is not a signed contract, proof of payment or permission to access every client system. If the approver is unknown, ask the client to name one before scheduling a final approval deadline. If an input is missing, record the missing item rather than completing the questionnaire on the client's behalf. Use the first-client-project.md checklist once scope and ownership are confirmed. ## Adapt the questions For a consultant, replace page count with the decision the engagement must support, interview participants and access to relevant business data. For a retainer, ask for the monthly deliverables, review cadence and what carries over. Avoid collecting sensitive records merely because the form has a notes box.
